While traditionally most medical offices have you fill out paper forms, we are 100% paperless. All of our paperwork is filled out on our website or via an iPad in our office. We request that you fill out the following form depending on who the visit is for:

WHAT TO EXPECT
On your first visit to Smile Team, we will review your dental and medical history form, your dental x-rays (if you have ones sent to us from another office) or we will take new x-rays if required. Then, Dr. Bansi Mithani Doescher will perform an exam and discuss all the findings and present all options for treatment (if treatment is needed). After that Dr. Doescher will answer all your questions or concerns. Finally, Dr. Doescher will perform a dental cleaning if no gum disease is present. To be as effective as possible, you may want to bring a list with your dental health questions to your visit to make sure they are addressed by Dr. Doescher.
In order to avoid unnecessary x-rays, please obtain any current x-rays from your previous dental provider before your appointment with Smile Team. X-rays should be emailed to team@gosmileteam.com. If you have any questions, please call our office at 215-709-0001.
We understand that your availability can change at the drop of the hat, but we kindly request that you provide the office 24 hours notice if you are unable to make your appointment. Please give us a call to reschedule.
